I thought the nightmares were excellently done - so vivid and genuinely disturbing. And I found Holmes and Watson's love story so, so moving. The scene where they woke up together in Watson's bed was tenderly erotic.
It was touching too that Mary wasn't overlooked in the story - Watson had loved her as much as he loved Holmes. And there was that little detail that Holmes thought of them as Watson and Mary - that perhaps he and Mary were friends; she wasn't just a former client and his friend's wife.
I thought the contrast between the nightmares and real life worked so well. The difference between the horror of the dreams and the undramatic, down-to-earth (though passionate) nature of the love between Holmes and Watson left me feeling a bit tearful.
